Animal Studies

Research involving direct interaction with an animal or group of animals, or the use of materials obtained from animals (e.g., their behavior or tissue samples).

Mandatory Forms:

• Checklist Form 

• Research Proposal Submission Form for Animal Studies

• Research Proposal

• Experimental Animal Use Form

• Progress Report for Medical/Scientific/Human/Plant Research 

• Final Report (Completion or Termination of Study)

• Use of Stem Cells, Fertilized Eggs, Gametes, and Embryos Form

• Device Usage Form

• Drug and/or Biological Products Usage Form

• Amendment Request for an Approved Project 

• Adverse Events / Serious and Unexpected Adverse Events Form

Research involving direct use of plants or plant groups, or samples of plant tissues.
